Monique Slee-Valentijn, MD, has previously worked as a hospital based physician in Internal Medicine. She subspecialized in Elderly and has always been fascinated by the (poor) organisation of healthcare (for the elderly). She is now the Director of 5 post-acute care services at Cordaan, the largest Home Care and Nursinghome-care provider in Amsterdam. To change the sytem she uses the methodology of Value Based Health Care. Leading principle is that the full cycle of care for a patient with a certain condition should be covered, by connecting all the professionals providing that care, no matter where they are based in the system. Her personal mission is to bridge the gap between Cure&Care. Having the same information and the right level of information contributes to providing better care. A technical platform can enable this. Together with the largest non-University Hospital, OLVG Hospital Amsterdam, the General Practitioners and their Healthcare Insurer Zilver Kruis, Cordaan initiated building the Virtual Ward. This Virtual ward is a technical platform that connects patients and professionals. At the HiMSS both the patient & professional perspective as the clinical & technical perspective and the required investments will be presented.
