To ensure patients are compliant with their treatments and adapt their lifestyles to their conditions or risks, health systems need to take into account the social determinants that may impact their behaviours. Integrating health and social care allows a more holistic views of patients and enables a better-coordinated and more person-centric approach. However, this integration is easier said than done. It requires a solid technology platform for sharing information between heterogeneous stakeholders and a strong alignment and commitment of health and social care professionals and caregivers. This session will present integration initiatives and solutions, discuss roadblocks and pitfalls, and highlight international best practices to overcome these challenges.
